Abstract

|
목적: 본 연구는 상급종합병원에서 혈액투석실에서 근무하고 있는 간호사들을 대상으로, 직무스트레스, 업무강도, 피로, 근골격계질환 증상 정도를 파악하고, 근골격계질환 증상 발생에 미치는 영향요인을 규명하기 위한 연구이다.
방법: 횡단적 조사연구로 13개 상급종합병원 125명의 혈액투석실 간호사를 편의표집 하였다. 자료는 SPSS/WIN 25.0 프로그램을 이용하여 t-test, χ2-test, factor analysis 및 binary logistic regression analysis를 이용하여 분석 하였다.
결과: 간호사 중 96명(76.8 %)이 근골격계 증상군이었고 29명(23.2 %)은 무증상군이었다. 증상군의 직무스트레스(p<.001), 업무강도(p<.001) 및 피로도(p=.011)는 무증상군에 비해 높았다. 혈액투석실 간호사의 근골격계질환 증상 발생에 영향을 미치는 요인으로는 직무스트레스(OR=1.13, 95% CI: 1.03∼1.25), 업무강도(OR=5.60, 95% CI: 1.25∼24.97), 취미 및 여가활동을 하지 않는 경우(OR=4.67, 혈액투석실 근무경력 2∼4년(OR=4.58, 95% CI: 1.04∼20.78), 혈액투석실 근무경력 8년 이상(OR=14.10, 95% CI 2.53∼78.64)인 것으로 나타났다.
결론 : 혈액투석실 간호사의 근골격계 질환 증상을 효과적으로 개선하기 위해서는 직무스트레스, 업무강도 감소 전략을 포함한 다차원적 중재 프로그램을 수립해야한다.

Purpose: This study was done to verify relationships among occupational stress, labor intensity, fatigue and musculoskeletal disorder symptoms, and especially to identify factors influencing musculoskeletal disorder symptoms among hemodialysis nurses in tertiary hospitals.
Methods: A cross-sectional design was used, with a convenience sample of 125 hemodialysis nurses from 13 tertiary hospitals. Data were collected through self-evaluation questionnaires during July and August, 2018. Data were analyzed using t-test, χ2 -test, factor analysis and binary logistic regression analysis with SPSS/WIN 25.0 program.
Results: Of the hemodialysis nurses 96 (76.8%) were in the musculoskeletal symptomatic group and 29 (23.2%) in asymptomatic group. Occupational stress (p<.001), labor intensity (p<.001), and fatigue (p=.011) were higher in the symptomatic group compared to the asymptomatic group. Logistic regression analysis showed that occupational stress (OR=1.13, 95% CI: 1.03∼1.25), labor intensity (OR=5.60, 95% CI: 1.25∼24.97), no leisure life or hobbies (OR=4.67, 95% CI: 1.48∼14.80), hemodialysis unit career 2∼4 year (OR=4.58, 95% CI: 1.04∼20.78), and hemodialysis unit career 8+ year (OR=14.10, 95% CI: 2.53∼78.64) were effect factors for the musculoskeletal symptomatic group.
Conclusion: For effective improvement of musculoskeletal disorder symptoms in hemodialysis nurses, multidimensional intervention programs are needed. Programs should include occupational stress, labor intensity and strategies to reduce symptoms.
